Overview of Tadalafil
Tadalafil is a prescription medication belonging to the class of phosphodiesterase type 5 (PDE5) inhibitors. It works by relaxing blood vessels and increasing blood flow to specific areas of the body. Clinically, tadalafil is primarily used to treat erectile dysfunction (ED) and the signs and symptoms of benign prostatic hyperplasia (BPH). In some cases, it may also be prescribed for pulmonary arterial hypertension (PAH) under a different brand. Tadalafil is distinguished by its long duration of action, often lasting up to 36 hours, earning it the nickname "the weekend pill." Conditions & Treatment Scope
Tadalafil is indicated for the management of the following conditions:
- Erectile Dysfunction (ED) – the inability to achieve or maintain an erection sufficient for satisfactory sexual performance. Tadalafil enhances the natural response to sexual stimulation.
- Benign Prostatic Hyperplasia (BPH) – a noncancerous enlargement of the prostate gland that can cause urinary symptoms such as difficulty starting urination, weak stream, and frequent urination (especially at night). Tadalafil helps relax smooth muscle in the prostate and bladder, improving urine flow and reducing symptoms.
- Pulmonary Arterial Hypertension (PAH) – a type of high blood pressure in the arteries of the lungs, which makes the heart work harder. Tadalafil (often under a different brand name) is used to improve exercise ability in these patients.

Important Safety & Precautions
- Contraindications: Do not take tadalafil if you are using any form of organic nitrate (e.g., nitroglycerin, isosorbide) or guanylate cyclase stimulators (e.g., riociguat). The combination can cause a severe and potentially fatal drop in blood pressure.
- Cardiovascular Risks: Sexual activity carries a potential cardiac risk. Tadalafil should be used with caution in men with pre-existing cardiovascular disease. It is not recommended for those who have had a recent heart attack or stroke, or those with unstable angina, severe heart failure, or uncontrolled arrhythmias.
- Priapism: Seek immediate medical attention if an erection lasts more than 4 hours. Priapism can permanently damage the penis.
- Vision & Hearing: Sudden loss of vision or hearing has been reported rarely with PDE5 inhibitors. Stop the medication and consult a doctor if you experience such changes.
- Alcohol & Grapefruit: Excessive alcohol consumption can increase the risk of side effects, such as dizziness and headache. Grapefruit products may increase tadalafil levels and should be avoided.
- Common Side Effects: Headache, flushing, indigestion, back pain, muscle aches, nasal congestion. These are usually mild and transient.
- Prescription Requirement: Tadalafil is not an over-the-counter medication. A thorough medical evaluation is necessary to ensure its safe use.
