Overview of Dapoxitine
Dapoxitine, known by its generic name dapoxetine, is a short-acting selective serotonin reuptake inhibitor (SSRI) specifically formulated for the on-demand treatment of premature ejaculation (PE) in adult men aged 18–64 years. Unlike other SSRIs used chronically for depression, dapoxetine is rapidly absorbed and eliminated, making it suitable for as-needed use 1–3 hours before anticipated sexual activity. It works by increasing serotonin activity in the central nervous system, which helps to delay ejaculation and improve control over the ejaculatory reflex, leading to increased intravaginal ejaculatory latency time (IELT) and enhanced sexual satisfaction.
Conditions & Treatment Scope
Dapoxitine is primarily indicated for men diagnosed with premature ejaculation—defined as persistent or recurrent ejaculation with minimal sexual stimulation before, upon, or shortly after penetration and before the person wishes it. It is approved for both lifelong (primary) and acquired (secondary) PE. The medication is not indicated for depression, anxiety, or other psychiatric conditions, and its use is limited to the management of PE under the guidance of a healthcare professional. Treatment may be combined with behavioral techniques and psychological counseling for optimal outcomes.
Important Safety & Precautions
- Contraindications: Dapoxitine is contraindicated in patients with significant cardiac conditions (such as heart failure, conduction abnormalities, and significant ischemic heart disease), moderate to severe hepatic impairment, and those taking monoamine oxidase inhibitors (MAOIs), thioridazine, or other serotonergic drugs due to the risk of serotonin syndrome. It should not be used concurrently with potent CYP3A4 inhibitors like ketoconazole or ritonavir.
- Common Side Effects: The most frequently reported adverse reactions include nausea, dizziness, headache, diarrhea, and insomnia. These are often transient and dose-dependent.
- Orthostatic Hypotension & Syncope: Dapoxetine can cause a drop in blood pressure upon standing, leading to dizziness or fainting. Patients should be advised to avoid situations where syncope could be dangerous and to rise slowly from sitting or lying positions.
- Serotonin Syndrome: Symptoms such as agitation, confusion, rapid heart rate, high blood pressure, dilated pupils, and muscle twitching require immediate medical attention.
- Alcohol Interaction: Combining dapoxetine with alcohol may increase the risk of neurocardiogenic adverse events (dizziness, somnolence, syncope) and should be avoided or strictly limited.
- General Precautions: Dapoxitine is not approved for use in women, children, or adolescents under 18. It must be taken exactly as prescribed, typically no more than one dose in a 24-hour period. Do not exceed the recommended dose. Patients should inform their doctor of all medications, supplements, and health conditions before starting treatment. It is not a cure for PE but a management tool; long-term safety data is limited.
